Early-life nutrition may play a bigger role in long-term brain health than previously understood, according to new research. A study published in the journal npj Aging suggests that limiting sugar intake during the first 1,000 days after conception—from pregnancy through approximately age 2—could be associated with a substantially lower risk of Alzheimer’s disease and other neurological conditions later in life.
The findings are based on health data from more than 60,000 participants and add to growing evidence that nutrition during infancy and pregnancy can influence lifelong health outcomes, including cognitive function.
Early-Life Sugar Restriction Was Associated With Lower Dementia Risk
Researchers analyzed data from 60,394 participants enrolled in the UK Biobank and found that individuals who experienced sugar restriction during the first 1,000 days after conception had a 46% lower risk of developing Alzheimer’s disease compared with those who were not exposed to similar restrictions.
The same group also showed a 27% lower risk of dementia from any cause, a 20% lower risk of anxiety, and an 11% lower risk of depression later in life.
The study suggests that dietary habits during pregnancy and early childhood could have lasting effects on brain health that extend well into older adulthood.
World War II Sugar Rationing Provided a Rare Research Opportunity
The researchers took advantage of a unique historical event in the United Kingdom. During and after World War II, sugar was heavily rationed nationwide. Children younger than age 2 were not provided a sugar ration, resulting in significantly lower sugar consumption during a critical period of development.
When rationing ended in September 1953, average adult sugar consumption nearly doubled.
Because intentionally restricting sugar intake in infants for research would be unethical, this historical period created what researchers describe as a natural experiment. It allowed scientists to compare people who experienced reduced sugar exposure early in life with those born after rationing had ended.
A research team in China examined health records for people born between 1951 and 1956. Participants were grouped based on whether sugar rationing occurred during pregnancy only, during pregnancy and early childhood, or during neither period, which served as the comparison group.
Researchers then reviewed medical records and self-reported health information from the UK Biobank to evaluate long-term health outcomes.
Brain Scans Suggested Slower Biological Brain Aging
To determine whether the differences extended beyond health outcomes, researchers also analyzed MRI scans from 9,053 participants.
After adjusting for factors such as genetics and family history, they found that individuals exposed to early-life sugar restriction had brains that appeared, on average, about 0.39 years younger than expected for their chronological age.
The MRI scans also showed larger volumes in brain regions including the hippocampus and thalamus, which are closely involved in memory, learning, and other cognitive functions.
Summarizing their findings, the researchers wrote: “These quasi-experimental findings suggest that early-life sugar restriction is associated with attenuated neurobiological aging and lower risks of dementia and psychiatric disorders.”
Experts Say Findings Are Promising but Not Proof of Cause
Professor Fahad Hanna, associate professor of public health at Torrens University Australia, told Newsweek that the findings align with current scientific understanding of early childhood development.
“The first 1,000 days represent a critical period of brain development during which nutrition can influence metabolism, immune function, the gut microbiome, and even epigenetic pathways that may have lifelong consequences,” Hanna said. “Excessive sugar intake early in life could contribute to metabolic dysregulation and chronic inflammation, and these are recognized as risk factors for cognitive decline and dementia.”
He emphasized, however, that Alzheimer’s disease develops through a combination of many factors rather than a single dietary influence.
“Alzheimer’s disease is a complex condition influenced by genetics, cardiovascular health, education, lifestyle, and many environmental factors accumulated across the lifespan. Early-life nutrition is likely to represent one piece of a much larger puzzle,” Hanna said.
The study authors reached a similar conclusion, noting that while they adjusted for numerous factors affecting brain health, the research does not establish that reducing sugar intake directly prevents Alzheimer’s disease or other neurological disorders.
Hanna also highlighted the limitations of observational research.
“The study provides an important association but does not establish direct causation,” Hanna said. He noted that people exposed to lower sugar intake early in life “may also have differed in many other ways that influence dementia risk decades later, including socioeconomic circumstances, health consciousness in general (including physical activity) healthcare access, education, and broader dietary patterns throughout childhood and adulthood.”
He added that the findings are based on a unique historical period of wartime sugar rationing and may not fully reflect modern eating habits in countries such as the United States, where processed foods and added sugars are more widely available.
The researchers echoed that caution in their paper, writing: “These findings require replication in contemporary cohorts with detailed early-life dietary assessments before informing clinical or public health recommendations.”
Public Health Guidance Still Supports Limiting Added Sugar
Although the study stops short of proving that lower sugar intake prevents Alzheimer’s disease, it reinforces existing public health recommendations encouraging moderation of added sugars during pregnancy and early childhood.
Hanna cautioned parents against overinterpreting the findings.
“Parents should not interpret these findings as suggesting that occasional sugar exposure determines a child’s future cognitive health,” Hanna said.
“Rather than viewing this as a story solely about sugar, I see it as further evidence that the first 1,000 days may be one of the most important windows for lifelong health. We have long recognized this for cardiovascular disease, obesity, and diabetes. Studies like this raise the intriguing possibility that brain health and dementia risk may also begin much earlier in life than we once thought.”
While additional research is needed before these findings influence clinical recommendations, the study adds to growing evidence that healthy nutrition during pregnancy and early childhood may have lasting benefits for brain health throughout adulthood.
